We are delighted to welcome you to our dental family, and we appreciate the opportunity to provide you with quality dentistry and a comfortable experience.  Our desire is to enter into a partnership with you based on trust and responsibility.  

In order that we may respond to your unique needs and concerns, we will ask that you complete a medical history and dental questionnaire.  If you have dental insurance, be sure to bring that information with you to your first visit as well.  

If you would like to finance your dental expenses, we would be more than happy to provide you with information from Care Credit or you can visit their website at www.carecredit.com.

You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment.
